Couples Therapy for OCD

OCD Doesn't
Just Affect
One Person.

When one partner has OCD, the other gets pulled into it. The reassurance loops. The accommodation. The household rules that multiply without anyone choosing them. The exhaustion that neither of you knows how to name.

Standard individual ERP works - but only if the relational cycle changes too. When a partner continues to accommodate OCD, it undermines the treatment. Recovery requires both of you.

Couples-ERP at The OCD Clinic teaches you to stop accommodating OCD and start fighting it together.

What's actually happening

The Cycle
Is Shared.

One partner seeks reassurance. The other gives it to keep the peace. The anxiety drops - briefly. Then it comes back, stronger, needing more. Both people are exhausted. Neither knows how to stop.

This isn't a failure of love or willpower. It's the OCD cycle operating exactly as designed - except now it's using your relationship as fuel. The more a partner accommodates, the more OCD demands.

Individual ERP is the treatment with the strongest evidence base for OCD. But when a partner continues to reassure, avoid triggers, or modify their own behaviour, it undermines the work.

The OUR OCD Method

A Framework
Built for Two.

Couples therapy for OCD at The OCD Clinic uses the OUR OCD method - a structured approach to breaking the relational OCD cycle. It's not talk therapy. It's a practical, behavioural framework that teaches both partners to stop accommodating OCD and start working against it together.

The OUR OCD Method is our structured approach to delivering ERP within couples work, built on established ERP principles and the research on family accommodation in OCD. It's how we do ERP with two people in the room, not a new therapy.

O

Owning

Both partners acknowledge their role in the cycle - without blame. You stop being victims of OCD and become active partners in recovery. Owning means seeing clearly: this is our challenge, not yours or mine.

U

Understanding

OCD is sneaky. When both partners understand how it operates in their specific relationship - where the loops are, how accommodation feeds it - they can start outsmarting it together instead of being pulled along by it.

R

Relational Recovery

Instead of OCD weakening your bond, you learn to use your relationship as a force for healing. This isn't just about reducing symptoms - it's about building a partnership that OCD can't get traction in.

Who This Is For

Patterns We
Work With.

These are the most common relational OCD patterns we see. If any of these feel familiar, couples therapy may be exactly what's needed.

Reassurance Loops

One partner constantly seeks reassurance. The other gives it, again and again, hoping this time it'll be enough. It never is. The asking increases. The resentment builds.

Avoidance by Proxy

The partner without OCD reorganizes their life around triggers - taking over responsibilities, changing plans, walking on eggshells - to prevent anxiety that was never theirs to manage.

Harm OCD & Hypervigilance

The person with OCD withdraws from parenting or physical closeness. The partner monitors, hovers, and compensates - reinforcing the idea that the feared person is genuinely dangerous.

Contamination & Control

Household rules multiply. The partner complies to keep the peace. Both people are exhausted and increasingly isolated - from each other and from the world outside.

Moral Scrupulosity

Endless seeking of moral or relational reassurance after perceived slights. The partner becomes a confessor - and eventually, burns out in that role.

Relationship OCD (ROCD)

Doubt about the relationship itself. Constant checking: Do you really love me? Is this right? The partner's reassurances fuel the doubt rather than reduce it.

What Sessions Look Like

What to
Expect.

01

Joint Assessment

We map the specific OCD cycle in your relationship - where it starts, how each of you responds, what gets reinforced. Both partners are part of this picture.

02

Education on OCD Mechanics

Both partners develop a clear understanding of how OCD works - and why accommodation backfires. This shared understanding is foundational to everything that follows.

03

ERP - Together

You learn how to support ERP without enabling it. The partner without OCD gets a clear, active role: not as accommodation provider, but as a teammate in exposure work.

04

Communication Skills

Practical strategies to break reassurance loops, set boundaries without conflict, and respond to OCD demands in ways that reduce the cycle rather than amplify it.

05

In-Person or Online

Sessions available in-person in Toronto or online across Ontario. Typically weekly, 80-minute sessions. Both partners attend together.

06

25+ Years of OCD Specialization
